The GP as the Gatekeeper to Specialist Care

In the Netherlands, your General Practitioner ('huisarts') is responsible for most primary healthcare needs. Before you can see a medical specialist (e.g., a dermatologist, cardiologist, or orthopedic surgeon) in a hospital or clinic, you nearly always need a referral from your GP.

This system ensures that:

• Less serious conditions are managed by your GP, preventing unnecessary specialist visits.
• Specialists receive patients whose conditions truly require their expertise.
• Healthcare costs are contained, as unnecessary referrals are minimized.

When a Referral is Needed (and Not)

A GP referral is generally needed for:

• All hospital outpatient appointments with medical specialists.
• Mental health specialists (psychologists, psychiatrists).
• Specific allied health professionals (e.g., some forms of physical therapy).

However, there are a few exceptions where a referral might not be needed:

• Direct access to a physiotherapist (though GP referral is often still advisable or required for insurance coverage).
• Emergency care: In life-threatening emergencies (dial 112), you will be taken directly to the hospital.
• Dental care: You can usually go directly to a dentist without a GP referral (though for oral surgery in a hospital, a referral is often needed).

The Referral Process

If your GP determines that specialist care is needed, they will issue a referral. This can be:

• Written: A paper referral letter that you take with you when you make your appointment.

Choosing a Specialist or Hospital

With a referral in hand, you have the right to choose your healthcare provider. Factors to consider:

• Location: Proximity to your home or work.
• Expat-friendliness: Some hospitals or clinics are more accustomed to international patients and have English-speaking staff or expat desks.
• Insurance Contracts: If you have a 'natura' policy, ensure your chosen specialist or hospital has a contract with your health insurer to ensure full reimbursement.

Costs and Insurance Coverage

Specialist care and hospital treatments are covered by your basic health insurance, but they do count towards your mandatory 'eigen risico' (deductible). This means you will pay the first €385 (2025 figure) of these costs yourself before your insurance starts to reimburse.

If you have a 'natura' policy and choose an out-of-network provider without prior approval, your reimbursement may be reduced (e.g., to 60-80% of the cost). A 'restitutie' policy offers more freedom of choice with full reimbursement (up to market rate) but usually comes with a higher monthly premium.
